What is the Black Maternal Health Pilot?
The Black Maternal Health Pilot is an innovative program aimed at improving maternal health outcomes for 150 pregnant people by addressing deep-rooted disparities in care.
Program Requirements
All pregnant mothers (regardless of stage in pregnancy)
• Each client will receive a total of $2,000 in cash card assistance to use as needed for their families. The cash cards will be distributed by the following schedule:
YEAR 1
• $500 upon completion of the Social Determinants of Health screening
• $500 December 2025
YEAR 2
• $500 upon completion of the Year 1 survey
• $500 upon completion of the pilot-Dec 2026
• Clients must participate in all 8 encounters with their Navigator
• Client must complete the Social Determinants of Health screening with their Navigator.
• Clients must participate in surveys about program services throughout the 2-year program.
• Receiving this cash card or any in-kind gifts from CARE, the Atlanta Community Food Bank and Lyft will not prevent you from receiving government assistance.
